Consent for Medical/Surgical Care/Emergency Treatment and Child’s Medical Information
Emergency health care providers care for nearly 22 million sick and injured children under age 15 each year (Center for Disease Control-CDC), the vast majority of whom have good outcomes. Everyone is only one step away from a medical emergency. Having a child’s medical conditions/allergies and caring medical providers names can help tremendously. Waiting to contact responsible parent/guardians can waste time and may effect outcome.
PLEASE-not only for us, medical providers BUT more importantly for your child complete complete a consent form for each child under age. Below is a suggested form by The American College of Emergency Physicians (ACEP)
